Note & Tell Privacy Policy
1. Scope
This policy applies to Note & Tell (Japanese name: からだを伝えるノート, the “App”). The App helps users organize rehab, pain, and wellbeing records they enter themselves so they can use those records in conversations with healthcare providers. It does not diagnose, treat, prescribe, or recommend exercises.
2. Information stored by the App
The App stores the following information on the user’s device:
- Exercise names, repetitions, precautions, notes, and attached photos provided by a healthcare professional
- Exercise completion records
- Pain intensity, location, timing, circumstances, and notes
- Fatigue, anxiety, falls, daily challenges, and free-form notes
- Questions for healthcare providers and their status
- Healthcare institutions, facilities, and staff
- The next rehab visit date
- App settings such as language and acknowledgement state
3. Storage, backup, and restoration
Health records, questions, rehab content, attached photos, and other in-app data are stored only on this device. The App does not store this data on a developer server or online storage and excludes it from iCloud or Google cloud backup and synchronization with another device. In-app data is not retained for use on a new device after an operating-system device-to-device transfer.
As a result, in-app data cannot be restored after the App is deleted, the user changes devices, or the device is lost, damaged, erased, or reset. The operator cannot recover it.
A paid-version purchase can be checked with the same store account used for the purchase. However, the paid entitlement and in-app data are separate. Checking a purchase does not restore health records, questions, rehab content, attached photos, or other in-app data.
4. External transmission
The App does not send user-entered health records, questions, rehab content, attached photos, generated PDF content, or PDF files to the operator, RevenueCat, advertising providers, or analytics services. It does not display advertising or collect usage analytics.
The App uses the Apple or Google store and RevenueCat to purchase and check the paid version. Purchase-related information required for those operations—such as product information, purchase status, and an app user identifier—is processed, but no health records or other in-app data are sent.
Health records and other in-app data are not sent when a user opens the privacy policy or support page.
5. External PDF sharing
When a user creates a PDF from a report and chooses an operating-system sharing option, the PDF is passed to the destination selected by the user. After sharing, the PDF is managed by the user and the selected destination. The App does not track where or how many PDFs are saved. An externally saved PDF is not a backup that can restore all in-app data.
6. Retention and deletion
In-app data remains on the device until the user deletes it. “Delete all records” in Settings removes health records, rehab content, questions, healthcare institutions, facilities and staff, attached photos, and the next rehab visit date. Deleted information cannot be restored.
Some non-health app settings, including language and first-use acknowledgement state, are not removed by “Delete all records.” Purchase status is managed by the store and is not removed by deleting records.
The operator does not receive in-app data and therefore cannot view, restore, or delete it on the user’s behalf.
7. Medical notice
The App does not provide medical judgment. Consult a healthcare provider if you notice concerning changes such as worsening pain, a fall, or strong anxiety.
8. Changes to this policy
This policy may be updated when the App’s features or data handling change. Important changes will be announced on this page.
